This was a monumental task for one person to undertake; I am proud to have successfully completed it – “The Knowelden Report” HMSO 1984. Initially I was required to negotiate with numerous eminent professionals, invariably my Seniors, to obtain the participation in the Enquiry/Study of 8 Centres – Sheffield, Newcastle, Oxford, Leeds, Manchester, Liverpool, Birmingham, and Edinburgh.
I was personally responsible for coordinating the activities of nearly 200 professionals to ensure uniform methods of Investigation into the circumstances of each of the 1076 infant deaths which occurred in the Study areas during the 3 year period 1/4/1976 – 31/3/1979.
To ensure data collection was as complete and comparable between centres as possible, I designed standard proformas to record information gained from interviewing the bereaved parents, studying the infants’ medical records from all sources in primary and secondary care, the maternal records, the locally-held case conference (which I also attended) into each death, and the enormous quantity of epidemiological data required for case/control analyses.
With the help of Dr Tony Stanton (Consultant Paediatrician), I classified and categorised each death according to the clinical presentation (if any) during the period leading to death, without knowledge of the death certificate information or necropsy findings, in order to avoid bias. Particular emphasis was placed on identifying potentially avoidable factors present prior to death, which could indicate ways of preventing deaths in future. The clinical presentations were subsequently correlated with the corresponding detailed necropsy findings of the parallel Pathology Study, which were obtained without prior knowledge of the pre-morbid history.
I received a great deal of praise and recognition for my part in this Study, published many papers and articles, both in the Medical and Lay press, not only in the UK but also in many other countries worldwide. I was invited to speak at numerous meetings and conferences, featured on the front pages of leading National Daily Newspapers and in 2 National TV Documentaries looking at deficiencies in the provision of Healthcare for Children, (eg “The Purple Line” – Granada TV.). Separate bibliography concerning this period of time is available if required.
Unfortunately I became aware of and disillusioned by the realisation that certain individuals within the Study were more “driven” by furthering their own careers than improving Healthcare for Children. In addition, as a caring clinician, I disliked being told how to treat children, by people who were not Doctors, based on financial considerations and political expedience rather than clinical need. Finally, the Department of Health insisted that the Study Report adopt a bland, conciliatory position that did not criticise or apportion “blame” to any group of individuals. I felt that this emasculated the Study findings that many of the infant deaths were NOT “Sudden and Unexpected”, but “Potentially Preventable” if the child had received appropriate (medical) care. As a combined result of these issues, I resigned from the NHS in May 1981.

“From scratch”, I have built up a very successful, full time, wholly private, Primary Care, “consulting only” medical practice.
I have always taken great care to keep “up to date” by attending numerous Conferences and Meetings, and remaining very aware of my limitations. My recent “Peer Reviews”, Practice Patient Questionnaires and many complimentary letters from Consultant Colleagues confirm that.
I have taken part in multiple annual Appraisals via The Independent Doctors Forum.
The practice has a reputation for being caring, ethical, and very professional. Currently we see almost exclusively children; nearly 15,000 (fifteen thousand) children have been seen since January 1st 2002. The majority of consultations have been the result of parents’ increasing unease about, and distrust of, the National Childhood Immunisation Program.
Parents have also developed an increasing distrust of their NHS General Practices, mainly as a result of the Practices’ lack of knowledge resulting in the inability to answer parents’ questions, but also because of the increasing numbers of children who are given inappropriate/incorrect vaccines, without parental consent.
The Practice provides a large amount of complementary factual written and verbal information about immunology, vaccine Schedules, injection techniques, and the medico-legal aspects of consent.
This information has been commended as being “factual and without bias” in our recent Healthcare Commission Inspection Reports.
I have needed to carry out a great deal of research and attend many specialised Conferences, Seminars and clinical meetings, to acquire the in-depth specialist knowledge about childhood vaccines that I needed to be able to provide detailed answers to parents’ questions, in a language that they could understand.
I have set up a website dedicated to provide information about available vaccines, the ever-changing Childhood Immunisation Programme, and the reasons for those changes.
I have published letters in several Journals regarding injection techniques for vaccinating children, and on request wrote the summary of the “Flagship Conference” on Childhood Immunisation for the June 2004 Royal Society of Medicine Journal.
To my knowledge, for at least 15 years I have been the only full time wholly private Primary Care Practitioner in the West Midlands. There appear to be only 12 such doctors in the UK, outside London. However, the success of this Practice depends on several Individuals working as a team, not the input from one person.
We have never advertised. Referrals come from other Doctors and Nurses, Hospital Consultants, JABS, IDF, BUPA, BMI, Nuffield Group, local Radio and TV etc, but MOST importantly from parents and patients who have recommended us to others after consulting with us themselves.
Aesthetic Medicine
It is always a challenge to me to acquire new knowledge of theory, and learn new techniques. In common with all areas of Medicine with which I have been involved, I am drawn because I care and have empathy for patients, and NOT because of potential financial gain.
During the last 10 years, I have undergone considerable training, and gained wide personal experience of the uses of Botulinum Toxin ‘A’ (Botox/Vistabel/Xeomin), Dermal Fillers (Juvederm Ultra, Belotero, Restylane etc), and Microsclerotherapy (Fibrovein).
All aspects of my Aesthetic Medicine treatment have been facilitated by my earlier experience working in Neonatology, which gave me the advantage of becoming very competent in positioning very small needles into very small places; ie the veins, arteries and intrathecal spaces of preterm babies often weighing less than 2 pounds.
The administration of many tens of thousands of infant vaccinations has refined my injection technique to the extent that we truthfully state in our Information material that babies/children VERY rarely cry during or following vaccine administration at our Practice.
My Aesthetic Medicine training/experience includes numerous competence based and assessed Advanced Courses/Master classes in Advanced treatments with Botulinum Toxin, Advanced Microsclerotherapy, and Advanced techniques in the use of Dermal Fillers. I was fortunate enough to undergo a half-day “one to one” Master Class with Prof Jean Carruthers on June 26th 2006.
